We take great pride in offering accessible services to patients of all ages, at six locations in Southern Minnesota and Northern Iowa. We are committed to providing highly trained physicians and support staff to give you the best specialty care possible.

Please allow 15 to 20 minutes before your first appointment to fill out the following forms and information that will be needed before your appointment. If time allows we will mail these forms to you to fill out and bring with you to your appointment.

Patient Registration Form - Gives us your basic demographic information, address, phone, and insurance information.

Notice of Privacy Policy - For your review.

Patient Health History - Gives detailed medical information past and present and lists current medications.

Financial Policy - Signature needed allowing us to bill insurance company and agreeing to be responsible for what insurance doesn’t pay.

Review of Systems - Gives details about specific body system and current symptoms.

Release of Information - Filled out and signed if other facility records are needed or if you would like to share your information with a family member.

INSURANCE INFO
The Center for Specialty Care and South Central Surgical Center are preferred providers with most insurance companies, representing the majority of health insurance plans in Southern Minnesota, Northern Iowa and Eastern South Dakota. Some of the insurance companies we participate with are:
