Please allow 15 to 20 minutes before your first appointment to fill out the necessary forms and information for your initial visit. If time allows we will mail the forms to you to fill out and bring with you to your appointment.

The Center for Specialty Care and South Central Surgical Center are preferred providers with most insurance companies, representing the majority of health insurance plans in Southern Minnesota, Northern Iowa and Eastern South Dakota. Some of the insurance companies we participate with are:
